low blood pressure

My sister has been on capd for a few weeks.  She has been having way low blood pressure readings. (for example 69/40, especially when standing) She has lost alot of fluid in the past 6 weeks.  She was on hemo-dialysis for 2 weeks prior to her going on capd.  She is very weak due to the low blood pressure, although she is more than pleased with capd.  We think it may be that she has been using too many 4’s and 2’s.  Will that do it?  Don’t mean to ramble on,but sure could use your input.  Thanks so much!!

How’s your sister’s iron, hematocrit(hct), hemoglobin(hgb), and red blood cells(RBC)? Sometimes or I say most of the time, with my son, if his blood test are low they could cause you to have low blood pressure, dizziness and feeling tired. Do she take Epo to help her produce more blood cells? When she’s in Hemo, they give this to her IV or dialysis line but with PD, you have to poke yourself. If it’s low and she need it soon, she will have to have blood transfusion then she will do the Epo. I hope this will help a little. carol b posted the following article in the Peritoneal Forum http://brumley.com/renal/pdboard.html Dated  : February 11, 192000 at 08:46:57 Subject: Re: low blood pressure http://brumley.com/renal/pdmessages/1620.html Why may I ask is she using 4’s and 2’s.  Is she full of fluid that she needs to get rid of?  I do pd 4 times a day and have been since 10/98.  I use 1.50 bags with each exchange.  The 2.5 and 4.25 bags take alot of fluid from the body and unless she has a very overwhelming amount of fluid build up, which by now I would think not, she should speak to her dialysis nurse or nephroligist as to why she needs the 2.50 and 4.25 strength bags. As for the low blood pressure I’m not too sure that the overtaking of fluid contributes to this or not. Hopefully someone else can relate to your situation there.  Did you try posting to the nurse page/ I am really interested in your response as to the amount of fluid she has built up and why she uses 2.5’s and 4.25’s. If she just started PD, her body will need to adjust. You also mentioned that she was on Hemo for two weeks before starting CAPD. Correct me if I am wrong, but did she have any warning that her kidneys were failing or did this happen suddenly? On CAPD the fluid restrictions are not as severe and perhaps she needs more fluid in her system. My husband when he first started didn’t drink enough and had low blood pressure and also was dehydrated, so it is a fine balancing act everyday forever. She should discuss this with her nurse and get help to fine tune her needs. If she is not overloaded then maybe they will discontinue the 4.25 and maybe mix up though the day the 2.5 and the 1.5. Check weight, blood pressure and how much she is drinking every day until she get balanced and then remain vigilant forever.

Beth-PD nurse posted the following article in the Peritoneal Forum http://brumley.com/renal/pdboard.html Dated  : February 12, 192000 at 11:11:06 Subject: Re: low blood pressure http://brumley.com/renal/pdmessages/1623.html Your sister should have some idea of what her ideal weight should be.  She needs to be weighing herself every AM to get some idea of how her fluid status is.  If she is below her ideal weight, she is getting dehydrated.  She is taking more fluid off than she is putting in. Symptoms of dehydration include: dizziness when standing, weakness, low BP and malaise.  She needs to STOP using 4.25% dianeal.  She needs to use 1.5% dianeal until BP returns to more normal range and body weight is where she wants it without having sx of fluid overload.  Also, is she taking BP medications?  It could be that she needs an adjustment there.  It is important for your sister to keep in close contact with her PD nurse at times like these.  She should be talking to her/him frequently, especially since she is so new to PD. I know her nurse would like to hear from her about this!  Good luck.

John McHale posted the following article in the Patients Support Forum http://www.he.net/~brumley/renal/patientboard.html Dated  : November 13, 1998 at 04:27:19 Subject: low blood pressure http://www.he.net/~brumley/renal/patientmessages/3908.html My wife, age 55, is on peritoneal dialysis as a result kidney failure after a bone marrow transplant in 1995.  She also has thyroid failure.  Before the transplant she was mildly hypertensive; now her blood pressure is seldom above 90/60, she is seldom out of bed and can only walk short distances before becoming exhausted. Despite going on dialysis and taking thyroid medication, her conditions has deteriorated steadily over the last 3 years. The low blood pressure is the immediate problem we want to solve.  Has anyone been in a similar situation?  What did you or are you doing?

I donated a kidney to my husband seven months ago and since the surgery my blood pressure is  85/55 as the lowest to 90/60 as the highest. Right after the surgery I was getting dizzy when getting up but after a while I got used to it. I think your wife is feeling this way as a combination of her dyalisis and probably the medications she is taking for her other problems. You should consult with a physician, but I think you should worry if her blodd pressure goes any higher. At least when it is low her kidneys are not getting any more damaged and her chances of heart disease sre reduced. I too was a little dizzy the day or two after surgery, but then everything was ok.  Its been 11 days now.  Still doing ok.  Taking less pain medication and not taking as many naps. :) Up all day now.  Actually washed dishes this morning. I hope you get some answers about her weakness.  I think you’re looking in the wrong place when you’re looking at the blood pressure. Just my thoughts.  My mom is on CAPD and has been for 16 years.
